Documents we collect from our carriers and WHERE we send this data and WHY!
W-9 FORM
W-9 Form is required because it has EIN Number, The Legal name of the company and the
Address where it is registered! Besides that it is an important document that is required when we are filling the carrier packet for a Company to get it set-up with the new Brokers in the market.
MC Authority
It is also required when we are filling the carrier packet. The main thing in this document is the MC number and the Address where the company is registered! Also the date of Authority when it was issued!
Certificate of Insurance (COI):
It is required because this is the proof that the company that is booking the load has everything insured. Besides that COI tells us that the Carrier is having the Amount that is required to book a load for example: $1 Million for the Auto Mobile Liability, Cargo Insurance for $100k. Brokers usually ask dispatchers to get a certificate of holder. A Certificate of Holder is required when broker wants surety that if there is any accident or the commodity is damaged he will be getting paid by the Insurance Company.
